Terms and Conditions for Pride Rehab & Performance

Welcome to Pride Rehab & Performance. By accessing or using our website or services, you agree to the following Terms and Conditions. Please read them carefully.

1. Services Offered

Pride Rehab & Performance provides physical therapy, personal training, performance training, and related wellness services.

Physical therapy services are provided by licensed healthcare professionals and are intended to evaluate and treat injuries, pain, movement limitations, and other appropriate conditions. Personal training and performance services are intended to improve fitness, strength, movement, and physical performance and are not a substitute for medical care.

Pride Rehab & Performance does not provide emergency medical services. If you are experiencing a medical emergency, seek immediate medical attention.

2. Booking & Cancellations

Appointments may be requested through our online scheduling portal or by contacting Pride Rehab & Performance directly. Appointment requests are subject to availability and confirmation.

We require at least 24 hours' notice for cancellations or rescheduling.

  • Late cancellations may be subject to a $25 late-cancellation fee.

  • Missed appointments may be subject to a $50 no-show fee.

  • Repeated late cancellations or missed appointments may affect the ability to schedule future services.

Additional scheduling and cancellation policies may be provided as part of your patient or client agreement.

3. Insurance & Payment

Pride Rehab & Performance offers both insurance-based and self-pay physical therapy services, as well as self-pay personal training and performance services.

For insurance-based physical therapy:

  • Acceptance of an insurance plan does not guarantee coverage or payment.

  • Patients are responsible for understanding their individual insurance benefits, including deductibles, copayments, coinsurance, visit limits, authorization requirements, and non-covered services.

  • Patients are responsible for amounts determined by their insurance plan to be their financial responsibility.

  • Insurance coverage and network participation may change over time.

For self-pay services:

  • Payment is due at the time of service or as otherwise specified in the applicable package, membership, or payment agreement.

  • Self-pay rates, packages, and memberships are subject to change.

Additional financial terms may be provided in the applicable Patient Agreement, Financial Policy, or training agreement.

4. Packages, Memberships & Refunds

Packages and memberships may be subject to separate terms regarding payment, expiration, cancellation, renewal, and use.

Except where otherwise required by law or stated in a specific agreement, payments for completed services are non-refundable. Refunds for unused prepaid services, packages, or memberships will be handled according to the terms of the applicable agreement.

Billing errors should be reported to Pride Rehab & Performance so they can be reviewed and corrected when appropriate.

5. Participation & Assumption of Risk

Physical therapy, personal training, performance training, and exercise involve physical activity and may carry inherent risks.

Clients and patients are expected to provide accurate information regarding their health, symptoms, limitations, and other information relevant to safe participation and to notify Pride Rehab & Performance of significant changes that may affect their care or training.

Participation in personal training, performance training, and other fitness or wellness activities involves the ordinary risks associated with physical exercise.

6. Results Disclaimer

Individual results vary based on factors including health status, condition, participation, consistency, lifestyle, and other individual circumstances.

Pride Rehab & Performance does not guarantee specific medical, rehabilitation, fitness, performance, or aesthetic outcomes.

7. Website Information

Information provided on this website is for general informational and educational purposes and is not intended to establish a provider-patient relationship or replace individualized medical evaluation, diagnosis, or treatment.

Submitting information through the website or scheduling portal does not guarantee acceptance as a patient or client or guarantee a particular appointment time.
